  • Driver's license (or other state-issued photo identification)
  • Social Security card (or W-2 or 1099 with your Social Security Number clearly visible)

  • Two most recent state and federal income tax returns (with all statements and schedules attached).

    • Public assistance benefit determination notice or verification letter
    • Worker's Compensation determination or verification letter
    • Social Security Benefit Verification Letter

  • Most recent statements for all mortgages and/or real property liens
  • Vehicle titles for all vehicles you have any ownership interest in

  • Most recent vehicle payment statement showing the monthly payment amount and balance owed
  • Car Insurance Declarations Page, which you can get from your car insurance provider (NOT the Certificate of Insurance normally carried in your car or wallet)
  • Most recent statements for your 401k accounts, IRAs, stock holdings, mutual funds, or other investment accounts
  • Residential Lease Agreement
  • While we always acquire credit reports for our clients prior to filing a bankruptcy case, we do still recommend submitting your most recent statement from each of your known creditors.
